Our latest research project builds upon research we completed in 2011 as part of an AHRQ-funded research project to develop an information model for the patient centered medical home (PCMH).
The AHRQ task order had the stated purpose of seeking to advance the PCMH concept by supporting a broad understanding of the PCMH as a series of interactions and associated information flows among patients and other PCMH stakeholders.
To successfully complete the project, we created an original framework to support the development of scenarios, process flows, and information flows used to describe the PCMH. The framework we developed defined the scope of the PCMH by linking associated principles and capabilities that are typically present. These capabilities are then linked to a set of scenarios and interaction diagrams that describe the healthcare services delivered by the PCMH. From the scenarios, process flows, and interaction diagrams we can isolate individual data elements. These scenarios, process flows and interaction diagrams can be used to facilitate a broad understanding of the PCMH among patients, providers, and health IT vendors.
A common reference framework was needed to ensure consistency in that understanding, while allowing different audiences (stakeholder segments) to focus on suitable levels of detail. For example, the process flows within a PCMH may be of little interest to patients but will be crucial for the provider and health IT vendor audiences. Similarly, the health IT vendor audience may give little weight to narrative descriptions of a patient’s experience, as such narratives do not provide enough specificity to design an IT system.
Initial Research Findings
The results of our initial research project are currently in AHRQ’s extensive review process for clearance, accessibility, and usability, in addition to final editing and preparation for posting on the AHRQ website. Because the reports are in a queue, along with reports and other products from many projects, AHRQ is unable to give us an estimate of when they will be posted.
However, AHRQ has graciously given us clearance to share copies of the reports as we submitted them. Once these reports have been published on AHRQ’s website, this page will contain copies of those final edited reports.
